Asterisked items in the Forms and
Applications and Plumbing area of the Code Information Handouts
menu are now available. If you have questions regarding policies,

The latest
version of Adobe
Acrobat Reader is required for reading some documents;
the Reader is available free from Adobe
by clicking on
the Adobe icon at below left.
Tips for
filling out Applications and Forms online (Only
the forms designated by **):
1. To navigate through forms, place the cursor on a blank field with
the mouse,
and click on it, or hit the Tab key to go forward,
Shift+Tab keys to go back.
If Enter is hit before reaching the end of the
document, the cursor will
automatically return to the beginning of the
document.
2. Check box blanks can be activated by hitting the space bar or by
clicking
the mouse button when the cursor is in that field.
3. Forms with numerical values involving calculations will be
calculated
automatically onscreen.
4. To fax a complete form or application, just print it from the
screen and
fax it to us as with a typed or handwritten
document.
