Upon receipt of your completed form, the enforcement officer for your neighborhood will investigate your complaint. Please read the Confidentiality Preference Statement and indicate your preference by initials. Should legal action be necessary to
resolve a violation the complainants name may be required and disclosed. This type of
disclosure rarely occurs. |
